**1. ASSUMPTION OF RISK**  

I understand that using aerial silks, dance poles, DJ equipment, or other entertainment apparatus involves inherent risks including but not limited to:  

- Falls or collisions  

- Equipment failure  

- Muscle strains or broken bones  

- Other unforeseen injuries  

I voluntarily assume all risks associated with participation.

**2. SAFETY RULES**  

I agree to:  

☐ Not use equipment while intoxicated  

☐ Follow all verbal safety instructions  

☐ Respect weight limits (250lbs per apparatus)  

☐ Ensure proper clearance (10ft ceilings for silks)  

☐ Not modify or adjust equipment  

**3. RELEASE OF LIABILITY**  

I hereby release MizCanaBiz Entertainment, its owners, and employees from any claims or demands for:  

- Personal injury or property damage  

- Medical expenses  

- Lost wages or other damages  

**4. MINOR PARTICIPANTS**
